Registration and Sports Clearance Information for High School Athletics
Dear Parents/Guardians:
In an effort to ensure all of our new and returning student-athletes are cleared to participate in their sport prior to the first official practice, please read this letter CAREFULLY and in its entirety taking note of the new procedures and forms.
ONLINE SPORTS REGISTRATION- All students must register for EACH SPORT SEASON |
ALL ATHLETES Create an Arbiter Registration account WAYNE HILLS WAYNE VALLEY Complete all of the required information Click on the link for the sports season in which your child will be participating

Pre-Participation Physical Exam Packet Submit the last page of the packet, the Medical Eligibility Form, to Health Office
Health History Questionnaire - complete and submit if the student athlete’s physical exam was completed more than 90 days prior to the first official practice date.

ImPACT® CONCUSSION BASELINE TESTING |
ImPACT® Concussion Baseline Testing is to be completed by ALL student athletes. The Athletic Trainers will schedule and administer the computer - based testing for all new and returning athletes. The ImPACT® testing is valid for two years. |
CLEARANCE TO PARTICIPATE IN ATHLETICS |
Once registration for the sport through rSchoolsToday is verified, the Medical Eligibility Form and the Health History Update Questionnaire have been reviewed and approved by the High School Nurse(s) AND the School Physician- a clearance letter to participate will be sent to the parent/guardian via email. As per NJSIAA- Failure to comply with the above due dates will prevent your child from participating in their sport Your child will NOT be permitted to participate in official practice without clearance. |
PLEASE SUBMIT FORMS AS EARLY AS POSSIBLE TO PREVENT POTENTIAL DELAYS IN THE STUDENT ATHLETE'S PARTICIPATION.
IMPORTANT REMINDER: Students who are excluded from PE participation for any reason are not permitted to participate in athletics
