ONLINE SPORTS REGISTRATION- All students must register for EACH SPORT SEASON

ALL ATHLETES

          Create an Form RELeaf Registration account     WAYNE VALLEY

          Complete all of the required information

          Click on the link for the sports season in which your child will be participating   

Please note: Only one account per family is needed. Please keep a copy of your username and password for future registrations.

HEALTH FORMS- Retained by your child's Healthcare Provider **MANDATORY**

1. Pre-Participation Physical Exam Packet

1. Submit the last page of the packet, the Medical Eligibility Form, to Health Office

2. Health History Questionnaire - complete and upload this form during the registration process.

PREPARTICIPATION PHYSICAL EVALUATION MEDICAL ELIGIBILITY FORM TO THE NURSE

HEALTH HISTORY QUESTIONNAIRE MUST BE UPLOADED IN FORM RELEAF

ImPACT® CONCUSSION BASELINE TESTING

ImPACT® Concussion Baseline Testing is to be completed by ALL student athletes. The Athletic Trainers will schedule and administer the computer - based testing for all new and returning athletes. The ImPACT® testing is valid for two years. 

CLEARANCE TO PARTICIPATE IN ATHLETICS

Once registration for the sport through Form RELeaf is verified, the Medical Eligibility Form and the Health History Update Questionnaire have been reviewed and approved - a clearance letter to participate will be sent to the parent/guardian via email. 

As per NJSIAA- Failure to comply with the above due dates will prevent your child from participating in their sport

Your child will NOT be permitted to participate in official practice without clearance.
